They are not in conflict, because KLAS surveys the organization that buys and administers the software, and the app stores capture the clinicians who are handed it. Which number matters depends on what you are buying for. If the job is to generate a defensible schedule across a large, complex organization and satisfy the people who administer it, Lightning Bolt’s record is excellent. If your problem includes what happens on three hundred phones after the schedule is published, the second number is the one to investigate. What Lightning Bolt is
Lightning Bolt is provider and staff scheduling software for hospitals, health systems, academic medical centres and physician groups. It was founded in 2002 in San Francisco and acquired by PerfectServe in February 2019, alongside the patient engagement company CareWire. At the time of acquisition it was described as managing more than three million physician hours a month. Its engine is described by Lightning Bolt as rule based automation using “an AI-enabled process called combinatorial optimization to identify the best solution, in this case a schedule, from a finite set of possibilities”, accounting for staff availability, skill sets and shift preferences across more than a hundred specialties. Its record is the strongest in the category. KLAS named PerfectServe Lightning Bolt Scheduling Best in KLAS for Scheduling: Physician in 2026 with a score of 90.8, following wins in 2024 and 2025. PerfectServe counts five in the category in total, adding 2017 and 2018, though those two were awarded under KLAS’s earlier Category Leader branding and predate PerfectServe’s acquisition of Lightning Bolt. Across scheduling and clinical communications, PerfectServe counts eleven in a decade. PerfectServe also publishes a customer case study in which the hospitalist group at UK HealthCare reduced the hours spent building provider schedules from 1,480 to as little as 260 per year. That is a claim in real units with a named customer, which is more than most vendors in this category offer, including us. We would rather point at it than pretend it is not there. It also integrates with Epic. PerfectServe publishes Epic On Call Finder and Epic OpTime integrations and has been on the Epic App Orchard since December 2020. It is worth knowing how: per PerfectServe’s own pages, the mechanism is a CSV file transferred over SFTP on a recurring basis, which is batch rather than live. If you are comparing vendors on Epic depth, compare the mechanism rather than the logo, and ask each of them the same question. What Lightning Bolt is not is a workforce operations platform, and the boundary is worth stating precisely because it is easy to get wrong in both directions. Lightning Bolt does feed payroll: its integrations deliver hours scheduled, hours worked and time off to systems like Kronos and Workday, and PerfectServe publishes a case study on exactly that. What it does not have is a native time and attendance product, a pay product of its own, or any monitoring of credential and licence expiry against the schedule. Credentialing systems appear in its integration list as an inbound source of contact and demographic data, not as something it tracks. PerfectServe’s own products are four: clinical communication and collaboration, medical answering service, provider and staff scheduling, and an operator console. That is a communication company with the best scheduling engine in the market attached, which is a different animal from a workforce operations platform.
How Lightning Bolt is rated, and by whom
This is the part of the evaluation most buyers miss, and it is worth doing carefully because it applies to every vendor here, us included. KLAS builds those scores by interviewing the organization: schedulers, administrators, IT and executives, the people who evaluate whether the engine produces workable schedules, whether implementation went well and whether the vendor answers the phone. On that population, Lightning Bolt is the best rated product in the category by a clear margin. App stores: 2.8 out of 5 from 147 ratings on the US App Store, 3.0 out of 5 from 93 reviews on Google Play. Those come from clinicians, who did not select the product, do not administer it, and interact with it as an app on a phone between cases. The recurring themes in the public reviews are being logged out repeatedly, time zone handling when syncing to a phone calendar, and general usability. Neither number is wrong and neither cancels the other. They measure different populations having different experiences of the same product, and the gap between them is information. It tells you that the engine and the vendor relationship are strong, and that the clinician facing surface is where the complaints live. Why this matters commercially: adoption problems do not show up in a procurement scorecard, they show up eighteen months later as clinicians who will not use the app, swaps that migrate back into group chats, and a scheduler doing coordination by phone again. If you are the person who will be blamed for that, ask every vendor on your shortlist, including us, for their app store ratings by platform, and ask to speak to clinicians rather than only to administrators.
What YouShift is
YouShift is a healthcare workforce operations platform that starts from the schedule. Coverage requirements, compliance rules and clinician preferences go in, and the engine returns complete schedule options that satisfy those rules, which the scheduler chooses between rather than assembles by hand. Everything downstream runs on the same engine and the same data. Swaps and open shift offers are validated by the rules that built the schedule, so a trade that breaks coverage or a duty hour rule cannot be made. Clock in and clock out reconcile against the shift that was scheduled. Clinicians can see what they will be paid before payroll runs. License and credential expiry are read against future assignments, so a gap surfaces months ahead. Staffing cost decomposes by site, team and period. Optimization is specialty specific, because an emergency department, an anesthesia group and a hospitalist service do not share coverage logic, shift structures or fairness constraints.

Who the product is designed to satisfy
Lightning Bolt is built to satisfy the organization, and it does that better than anything else in the category, which is what a 90.8 measures. The clinician app is a way to view the schedule the organization produced. YouShift treats the clinician as a second user with their own job to do: submitting availability, trading a shift without asking permission from a group chat, picking up an open shift, checking what a month will pay before payroll runs. The reason is not generosity. A schedule that clinicians will not engage with pushes every subsequent coordination problem back onto the scheduler, and that is the cost that shows up eighteen months after go live rather than in the evaluation. You do not have to take our word for which approach works. Ask your own clinicians to use both.
Which way the platform extends
Both companies are building beyond the schedule, in different directions. PerfectServe extends sideways, into clinical communication: secure messaging, answering service, patient engagement. If your organization wants scheduling and communication from one vendor, under one contract and one integration, that is a coherent and genuinely attractive proposition, and no scheduling-only product matches it. YouShift extends downward, into what the schedule causes: maintenance under the same rules, timesheets reconciled to scheduled shifts, pay, credential expiry against future assignments, and the cost of coverage decomposed by site and period. If your unsolved problems are on that axis rather than the communication axis, the suite that includes messaging does not help you. Neither direction is better in the abstract. They are answers to different questions, and you already know which question you have.
What you buy, and how you test it
Neither company publishes pricing, and both quote per organization. YouShift charges for clinicians actually being scheduled rather than for everyone on the roster, which matters most if you carry a long tail of per diem and locum coverage. The difference is how you find out whether it works. Lightning Bolt publishes no free trial. The evaluation is a demo, references and an implementation. YouShift runs free for a full scheduling period, on your own coverage rules and your own clinicians, so the question of whether the engine handles your rule set and whether your clinicians will use the app is settled by observation rather than by a scorecard. Given that the KLAS score and the app store ratings point in different directions, this is not a small procedural detail. What Lightning Bolt costs
Lightning Bolt does not publish pricing. Licensing is per seat, quoted per organization, and there is no published free trial: the evaluation path is a demo and a product tour. Reviewers have described initial pricing as high, though the public review base is small and dated enough that we would not build an argument on it. Per seat is worth noticing next to how we charge. A per seat licence bills for everyone who has an account, and YouShift bills for clinicians actually being scheduled. For a group with a long tail of per diem and locum clinicians who appear on the roster but on few shifts, those two models produce very different invoices, and it is worth modelling both against your own roster rather than comparing headline rates. If you are comparing quotes, ask for a three year total including implementation, and ask specifically what is in the scheduling contract and what would require adding PerfectServe’s communication products, since the suite is where the pricing conversation usually goes. Two questions: what happens to your history, and what happens to your rules. Historical schedules and clinician records are migrated as part of onboarding. The rules are the real work, and with Lightning Bolt there is a specific consideration. A rule set tuned inside a combinatorial optimization engine over several years encodes priorities and weightings that were tuned rather than written down, and the people who tuned them may have moved on. Budget time to restate the intent rather than to copy the configuration. On timing: switch between scheduling periods, never inside one, and run the first generated schedule in parallel against the one you would have produced. The free scheduling period exists for exactly this. If the output does not survive the comparison, do not switch, and if you decide not to continue, ask and the YouShift team will give you your data, then delete it.
